1) A camp brochure for each camp date is required with the application.
2) Coverage is not available for camps with adult participants.

For fastest coverage and service, we recommend that you use our online application. You just fill out the online application and enter your check information to submit the request to us. Then fax your camp brochure to (803) 256-4017. Within 24-48 hours we will review and approve your application and brochure and you will receive your Proof of Coverage documents in your email. (If for some reason we are unable to approve your application, you will be notified by email.)
