Add & Delete Forms   Use these forms to increase or decrease the number of teams in your league. Please print and complete the forms when changes are necessary. We hope you will find it convenient as these forms contain all the information necessary for us to complete your request on a timely basis.

All requests to delete teams must be received by Sadler & Company within two months of the date the original application is received by Sadler & Company.
