Add & Delete Forms Use these forms to increase or decrease the number of teams in your league. Please print and complete the forms when changes are necessary. We hope you will find it convenient as these forms contain all the information necessary for us to complete your request on a timely basis! All requests to delete teams must be received by Sadler & Company on or before 10-1-03. Please note: There are two forms which must be completed to amend your number of teams. One will amend your insurance policy, and one will amend your AYF charter. Please note that the computer that you are using must have Adobe Acrobat Reader version 4.0 or higher.
